Chemical peeling

Chemical peeling အရေပြားချွတ်ခြင်း

Different types of Chemical peels

(1) Light chemical peel

  • removes the outer layer of skin (epidermis)
  • used in fine wrinkles, acne, uneven skin tone and dryness
  • Can be done every two to five weeks — depending on your desired results.

(2) Medium chemical peel

  • removes skin cells from the epidermis and portions of the upper part of middle layer of skin (dermis)
  • Used in wrinkles, acne scars and uneven skin tone
  • Can perform after 3-9 months to maintain results

(3) Deep chemical peel

  • removes skin cells from the epidermis and from portions of the mid to lower layer of dermis
  • Used in deeper wrinkles, scars or precancerous growths
  • can only be performed once

Indications for various diseases

For acne

glycolic acid (GA), salicylic acid (SA), β-lipohydroxy acid (LHA), Jessner’s solution

For acne scar

SA, GA, TCA, LHA, Jessner’s solution, phenol 

Photodamage 

SA, GA, TCA, LHA, Jessner’s solution, phenol 

Melasma 

GA, TCA, resorcinol (50% solution)

Scarring

Skin color changes

Infection 

Heart and liver damage (due to phenol)

Management

Before procedure

clean the face and cover eyes with ointment, gauze, tape or goggles. 

Can protect the hair with band

Local anaesthetic need in deep peel, while medium peel just need painkiller

During procedure

Light peel

  • brush, cotton ball, gauze or sponge to apply a chemical solution (glycolic acid or salicylic acid)
  • The skin will begin to whiten and the patient feel mild stinging
  • Can apply neutralizing solution or wash to remove the chemical solution

Medium peel

  • cotton-tipped applicator or gauze to apply a chemical solution (trichloroacetic acid)
  • The skin will begin to whiten.
  • After a few minutes, cool compresses will apply to soothe the skin. 
  • The hand-held fan require to cool the skin
  • The skin may feel stinging and burning for up to 20 minutes.
  • No neutralizing solution needed

Deep peel

  • cotton-tipped applicator to apply phenol (carbolic acid) 
  • The skin will begin to turn white or gray
  • The procedure will be done in portions at 15-minute intervals (the whole-facial procedure take about 90 minutes)

After procedure

cleansing, moisturizing and protective ointments should be done carefully as instructed by doctor.

After a light chemical peel – the healing will take 1-7 days, and should apply protective ointment (petroleum jelly)

After a medium chemical peel – the healing will take 7-14 days, but redness may last for months. At first, treated skin will form a crust and develop brown blotches. Give OTC pain-killer and apply protective ointment (petroleum jelly)

After a deep chemical peel, feel burning, throbbing, severe redness and swelling

Stay at home while healing process and covers the treated area about two weeks.
